Immunophenotypic characterization of cholangiocarcinoma: a review of 67 cases diagnosed at the Contreras Robledo Pathology Laboratory over the past five years

Cholangiocarcinoma (CCA) is a malignant neoplasm of the biliary epithelium whose differential diagnosis from other primary hepatic neoplasms and hepatic metastases is often challenging. In this context, immunophenotypic analysis is an essential tool for its characterization. Objectives: Characterize the immunohistochemical profile of cholangiocarcinomas diagnosed at the Contreras Robledo Pathology Laboratory over the past five years. Methods: A descriptive, retrospective study was conducted between January 2020 and August 2025. Demographic and morphological variables, as well as immunophenotypic findings, were evaluated. Results: A total of 67 cases were identified, with a female predominance (61.1%) and a mean age of 61 years (range: 16-89). The CK19 marker was positive in all evaluated cases. The most frequent immunophenotype was CK7+/CK20-, observed in 92.8% of cases. Hepatic markers Hep-Par1 and Arginase-1 were negative in 95% of cases. Conclusions: CCA exhibits a characteristic immunophenotypic profile. The combination of CK7+/CK20- and the absence of hepatocellular markers and/or other metastatic markers is useful for the differential diagnosis from other hepatic neoplasms and metastases from other primary tumors.
